New Patients

We gladly accept new patients to our clinic.

To assist our admin staff,  please click on the link below to access our new patient registration form.

If you could please complete this form prior to your visit and bring this with you to your appointment.

This registration form also includes a medical history form which will be required by your GP.  This form is necessary to ensure your GP has an insight to your medical history and to ensure you receive the best possible care from us.

Please navigate to our fees page to find out what your visit cost may be and our payment methods.

We ask all new patients to arrive 10 mins prior to their appointment time.  This will ensure we allow enough time to gather your information before seeing your doctor.

Also don't forget that the parking and entrance into our clinic is at the rear of the building.  

Just follow the parking signs.
